Nurses
Provides prescribed nursing care services to ill, injured and disabled persons in a home setting functioning independently without on-site supervision. Demonstrates knowledge and competency in performing baseline-nursing skills. Maintains clinical competencies according to Hospice standards. Communicate effectively with patients' families and physicians. Performs accurate assessments, develops and implements a written plan of care with the patient, following physicians' orders and according to Hospice guidelines. Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change. Demonstrates organization and leadership abilities.
Social Worker
Evaluates and assesses patient needs for social services assistance, planning and coordination of patient programs and acts as a liaison between the patient, hospital, Hospice, physician and community in the home care with appropriate linkage of community services/resources. Performs under supervision of MSW. Plans and conducts short-term goal-oriented psychotherapy for patients and families. Interviews patients and families in order to assess psychological, social, emotional and economic needs.
Clinical Support
Assists with patient billing. Responsible for department filing. Coordinates activities of other collection personnel. Answering phone. Coordinating schedules. Promotes customer satisfaction.
HHA
Provides personal care and related services in the home. Functions under direct instruction and supervision of an RN.
Assists patients with personal care to preserve good personal hygiene while maintain a healthful, safe environment. Establishes and maintains a good working relationship with the patient/family and co-workers. Report any changes in patients' condition or needs to the Supervisor or RN. Must have HHA certification.
Chaplain
Provides counseling services and spiritual support to patient and families. Develops and implements programs to meet needs of families before and after the death of the patient. Coordinates implementation of the bereaved's plan of care. Participates as a member of the management team in planning, developing, implementing, and evaluation policy and procedures. Collects data and documentation for bereavement.
Sales
Responsible for creating and promoting visibility and positioning of the Hospice in the healthcare community. Builds volume for home care services in a way that supports the philosophy and enhances the image of the Hospice. Maintains contact with representatives of other organizations to exchange and update information on resources and services available. Works to strategize, plan and supervise marketing communication activities. Prepare marketing plans.
